For a planned, non-emergency case

Detailed Planning Questionnaire

CHC has not abandoned the longer questionnaire. It is used after a human Scope Preview when a patient or family needs to organize a more complete, structured planning brief before deciding whether to buy the China Visit Planning Review.

Structured, not clinical

It organizes planning facts. CHC does not diagnose, recommend treatment, rank providers, or decide whether you are fit to travel.

Human-gated

A CHC coordinator first confirms whether deeper planning may be within scope. The questionnaire alone does not create a paid order or provider inquiry.

No files here

No uploads, attachments, passport copies, raw imaging, or payment details belong in the detailed questionnaire.

What the longer questionnaire covers

  • ✓Your planned service, travel window, and maximum stay in China
  • ✓The question you want a coordination review to address
  • ✓Whether you already have reports, imaging, or a local quotation — without uploading them
  • ✓Language, mobility, travel, and practical support considerations
  • ✓Your preferred contact method and whether you want a human follow-up

When materials may be requested later

Only after CHC has confirmed a paid scope, explained the materials needed, obtained explicit consent to process health information, and — where relevant — obtained separate authorization to share selected materials with a named provider. A human coordinator activates the materials process; this page is not an upload portal.

New to CHC? Start with the short public inquiry. If a longer brief is appropriate, a coordinator will explain the next step.
