Controlled case materials

CHC does not invite patients to send full medical records, imaging, passports, or payment information through public forms, WhatsApp, or ordinary email. A human coordinator activates a separate case-materials process only when it is necessary and authorized.

Five gates before materials are requested

  1. 1A CHC coordinator has confirmed a written, paid scope.
  2. 2The patient has explicitly consented to CHC processing the minimum health information needed for that scope.
  3. 3If a provider inquiry is relevant, the patient has separately authorized sharing selected materials with a named provider.
  4. 4A CHC human owner has reviewed the case and approved the next administrative action.
  5. 5An access-controlled case workspace has been activated by CHC. This website is not that workspace.

Minimum necessary

CHC requests only the particular reports, summaries, or provider paperwork needed for the agreed administrative scope. It does not routinely ask for full records, raw imaging, or identity documents.

Human-controlled sharing

A CHC team member selects and sends any authorized material. CHC's automated tools do not transmit records, choose a provider, or make clinical decisions.
