For patient or family inquiries that appear to fit CHC's non-clinical coordination scope, the next paid step is the Readiness Assessment for record organization, missing-materials review, budget planning, and coordination boundaries.
Concierge Scope Confirmation
Concierge Coordination is scoped separately only after the Readiness Assessment has been completed. Completion of the assessment does not guarantee that CHC will offer Concierge Coordination.
Coordination & Wayfinding
Concierge Coordination can include provider administrative inquiry, appointment logistics, interpreter arrangement, hotel and transport coordination, and practical local wayfinding.
Post-Visit Documents
After the visit, CHC can help coordinate report collection, translation coordination, provider-message routing, and return-home document handoff when included in the service scope.
